Payment Schedule
We offer convenient automatic payment options on a monthly or quarterly basis, with all charges processed on the 5th of each month. You may choose from two available options.

We accept VISA, MasterCard, Discover, and American Express. To use this option, please download and print the ‘Credit Card Authorization Form’ PDF below. Once completed, return the form to us via fax, email, or mail.
Credit / Debit Card Form
Bank Account (ACH)
Direct charges to your checking account. To choose this option, please download and print the ‘Bank Account Authorization Form’ PDF below. Once completed, return the form along with a voided check to us via fax, email, or mail.
